Welcome to the consulting page. Here is where you begin the process of scheduling an appointment to discuss your gut issues with me.
Keep in mind that this will only be a conversation, and under no circumstance can I diagnose or treat your medical condition. Only a licensed M.D. can legally do both.
I’ll ask you a ton of questions so I can understand what you’ve tried, who you’ve seen and where you are now. I’ll use this information to figure out what, if anything, might have been missed and what your next best course of action should be.
If you do decide to talk to me, you will need to complete two forms. The first is a disclaimer where you acknowledge that this is only a friendly chit chat and that you fully understand that I am not a medical doctor.
The second is a health questionnaire about prior testing, symptoms you’re experiencing, past treatments, supplement & drug use, diet, etc. Both forms come in PDF format and can be filled out on your computer or tablet device without the need for printing.
If you aren’t comfortable sharing this information with me, then it might be best if you seek advice elsewhere as I really need to know this information to have any hope of helping you. You will be able to view the questionnaire before finalizing payment to decide if you want to entrust me with this information.
The cost for my time is $50 for a one hour Skype/FaceTime video call. I don’t do email-only consults as I really need to talk to you to get an in-depth idea of what’s going on.
Both forms must be in my possession at least 24 hours before your consult. That gives me time to adequately prepare for our meeting.
Once an appointment time has been agreed to, an invoice for payment will be issued via email through PayPal. A PayPal account is not required. Payment must be made at least 24 hours before your scheduled session.
My time is limited so please understand if I can’t arrange a talk as soon as you wish. Also, keep in mind that I’m located in Southern California. That means that time differences may make scheduling an appointment difficult if you’re located outside the United States, but I will try to make it work.
If you wish to schedule an appointment, you can email me at email@example.com.
